Mandatory updating of the COVID-19 tracking system by citizens Announcement of the Ministry of Health What those who are detected positive should know

Due to the increased number of tests that result in the detection of a large number of positive cases, the Ministry of Health announces to citizens that allconfirmed cases are now required to complete an online form to better serve them and facilitate the tracing process.

As a reminder, citizens receive a text message on their mobile phone the day after their diagnosis[/B] that they indicated when they were sampled. The message contains a link with passwords to confirm their details, to register their contacts and their personal physician so that they can be monitored and released, and to be able to issue the necessary release certificates at the end of their self-limitation period.

The personal online form can be completed by the citizen, or in the case of minors by the parent/guardian, via smart phone, tablet or computer at https:/phase.ucy.ac.cy/, receiving passwords which are valid for 48 hours from the time of receipt of the sms. Within these 48 hours the citizen can edit and save the electronic form, and subsequently submit it to the Ministry of Health by pressing the registration button. It is noted that if the electronic form is not completed correctly, then the personal physician will not be able to release the case and the person will not be able to receive a certificate of recovery from the Republic of Cyprus.

Completion of the electronic form is considered essential to ensure release, declaration of close contacts and notification of the personal physician in a timely manner so that there are no delays through the tracing process and telephone interviews due to increased workload.

For instructions on how to complete the online form, people can, after receiving the link and passwords, contact 22514241, 22514242, 22514243, 22514244 and 22514248 (Monday to Sunday 08:00-00:00).

If a person does not receive a text message with the passwords and link within 24 hours of their test, they can contact the Tracing Team at 22514264, 22514258, 22514259, 22771923 (Monday to Sunday 08:00-00:00) or email contacttracing2@moh.gov.cy or the laboratory where the test was carried out, to check that his/her details were sent to the Ministry of Health
